Documents required for Fresh Passport
Government address proof required such as Aadhar Card, Voter ID, Bank Passbook etc. (Any 02)
Birth Certificate, Aadhar Card, PAN Card, Voter ID etc. (Any 01)
Matriculation(10th Passing Certificate or 10th Mark Sheet) or any other Higher Educational Pass Certificate which has DOB(Date of Birth). (Any 01)
If any person is illiterate or having education qualification less than 10th(Matriculation) then he/she must have to provide PAN Card.
1) Present address proof document in the name of parent(s) should be required.
2) Birth certificate may be required.
Aadhar Card, Voter ID, Bank Passbook and Pan Card. (Any 03)

Important Note: Fingerprints and facial photograph are required at Passport Seva Kendra (PSK). Personal appearance as per the appointment is therefore mandatory. On the day of the appointment, the applicant has to carry all the original documents with one set of self-attested photocopies of the same at the Passport Seva Kendra (PSK) for processing.
